If the "captain" of the ship is a LT, then how should one address her/him? Capt? Lt? Ma'am?

Either Captain, LT, or Mr/Ms ______ will work.
You can meet the standard, or you can set the standard. It's your choice.

Colin's CO is a woman... I know a man some will refer to him as the "Old Man"... would you dare refer to a female CO as the "Old Woman"?

the co of the vessel usually lets the crew know how they would like to be addressed. if my co wasn't a CAPT, i always referred to them as sir. but that was just me.

During my meeting with the CO(a LT as well) of the cutter I was on I asked him what he prefered to be called.
If you were hoping for the A Team you will be sorry to know you got the F Troop.
